This poster states:
Carry out pre-use checks on Forklift Trucks - Every year approximately 10 fatalities are reported as a result of forklift operation. Employers and employees must ensure that a forklift truck is safe for use. Employers have a duty of care under PUWER (The Provision and Use of Work Equipment Regulations) to ensure all employees operating or working in the vicinity of forklifts are adequately informed of the potential hazards and that the relevant safe work procedure is in operation.

Fork Arms / Attachment No cracks or distortion. Evenly spaced on plate and clips engaged.
Carriage Plate No obvious damage. Square to mast. End stops secure.
Mast No damage, distortion or cracks. Inner channels or runners reasonably clean and smooth.
Lift Chain & Pulleys All pins / links in place, free from damage / rust and suitably lubricated.
Hydraulics / Controls No damage or leakage. Check fluid level. Operating smoothly.
Mast Controls Lift / lower, tilt and side shift operating properly.
Wheels & Tyres All nuts tight. No excessive wear, cuts or foreign bodies. Check pressure.
Back Rest / Load Guard Secure with no excess damage or distortion.
Seat & Seat Belt Correctly fixed to truck, not loose or damaged, secure and functioning.
Steering Works properly in both directions at standstill and when moving.
Driving & Braking Works properly in both directions when tested slowly.

How it Works
Attach our Universal Status Tag to the entrance of the Forklift Truck.
Use the Forklift Truck check book to carry out a comprehensive inspection.
Complete the inspection and record the assessment findings.
Remove the top sheet and insert into the Universal Status Tag.
Secure the Universal Status Tag by fitting a Tamper Evident Seal.
The Tag will now display the date and findings and advise employees.
