Mother grieves loss of son and HSE deem accident preventable

A 24-year-old engineer was fatally crushed whilst attempting to repair a factory goods lift with his colleagues on the 14th January 2020.
Lift Monitoring Systems Limited, previously known as RJ Lift Services Limited, was fined £200,000 after a HSE (Health and Safety Executive) investigation into the death.
The following information was found during the HSE investigation:
Lewis McFarlin (the victim) and two other lift engineers were on site to work on a different lift before being asked to resolve an issue with the door-opening mechanism on one of the lift landing doors.

Farmers are being reminded they must change their attitude towards safety as Britain’s workplace regulator readies itself for a wave of inspections in the coming months.
Inspectors from the Health and Safety Executive (HSE) will visit farms across England, Scotland and Wales as part of a push to change the culture in the industry and check for compliance with long standing legal requirements.
People on farms are 21 times more likely to be killed in a workplace accident than other industries.
In total, there have been 161 deaths on Britain’s farms over the last five years – an average of 26 people each year. This includes members of the public and children.

DVSA Van Statistics

The UK uses more that three and a half million vans for business activities. As part of targeted roadside checks, the DVSA (Driver and Vehicle Standards Agency) stop around 15,000 vans each year to carry out spot checks on vehicles and issue prohibitions if necessary. A prohibition prevents van drivers from using the vehicle until the problem is fixed and can cost owners and operators up to £4,000 per day, per vehicle.
59.9% of vehicle prohibitions were due to a mechanical defect / failure and 83.9% of vans were overloaded.
The top three mechanical defects for light goods vehicles that resulted in a prohibition were: 1. Tyre condition 2. Lights and indicators 3. Steering linkage.

Nearly 4000 trailers (including 730 caravans) were checked by the Driver and Vehicle Standards Agency (DVSA) between 2019 and 2021.
The inspections carried out by the DVSA highlighted significant issues with many of the trailers and caravans. Of the 3,083 trailers (not a caravan) that were inspected, 50% were non-compliant, and 41% were issued with an immediate prohibition (defects so dangerous that repairs are required prior to re-use).

The UK Material Handling Association have recently released their latest Safety Alert which looks at cold starting an LPG truck engine.
This Safety Alert has been issued following some reports of users of LPG powered trucks experiencing issues with the start-up process. An investigation by the Health and Safety Executive (HSE) found that a manufacturer of paper and cardboard (Amberset Limited) failed to regularly review safety measures on their machinery and equipment to ensure that access to hazardous parts was prevented. The HSE found that the safety of the paper rewinder had not been reviewed since August 1998.
If Amberset Limited had conducted safety inspections on their machinery and equipment they would have spotted that the paper rewinder was not suitably guarded and allowed access to dangerous parts at various places including the front of the rewinder where the employee was drawn into the machine by their hand between two exposed rollers.
